Job Description:
Schedule: Full time, Day shift, Monday through Friday, 8am-4:30pm
Assignment Duration: 13 Weeks
Compensation:
This position offers a daily rate of $700 to $750 which equates to $3,500 to $3,750 per week (based on a 5-day workweek)
Per Diems: Lodging: $191 per day and Meals & Incidentals: $86 per day
Per diems are paid daily, 7 days a week, totaling $1,939 per week
Total Weekly Compensation (Daily Rate + Per Diems): Between $5,439 to $5,689 per week

Summary:
The Case Management Manager oversees and directs case management operations within a specified region or facility. This role is responsible for ensuring the delivery of high-quality, cost-effective, and patient-centered care by leading a team of case managers, collaborating with interdisciplinary teams, and implementing strategies to improve patient outcomes and operational efficiency.
Essential Functions:
Lead, manage, and develop case management staff to ensure optimal utilization of human resources and achievement of financial, service, and clinical quality outcomes. Responsibilities include recruiting, hiring, training, evaluating performance, and addressing disciplinary actions when necessary.
Plan, implement, and evaluate case management programs and services to meet organizational goals and objectives. Develop and maintain policies and procedures that ensure compliance with regulatory requirements and industry standards.
Monitor and assess the effectiveness of case management services through data analysis and outcome measurements. Implement quality improvement initiatives to enhance service delivery and patient satisfaction.
Work closely with physicians, nurses, social workers, and other healthcare professionals to coordinate patient care across the continuum. Facilitate communication between various departments to ensure seamless service delivery.
Develop and manage departmental budgets, ensuring efficient allocation of resources. Identify opportunities for cost reduction while maintaining high-quality care standards.
Ensure that case management practices comply with federal, state, and local regulations, as well as organizational policies. Prepare for and participate in audits and accreditation surveys.
Qualifications:
Minimum of five (5) years of case management and/or disease management supervisory experience.
Bachelor's degree in nursing or a healthcare administration-related field.
Registered Nurse License (California).
Strong leadership, communication, and interpersonal skills. Proficiency in data analysis and utilization management. Knowledge of regulatory requirements and healthcare industry standards required.
Case Manager Certification (CCM) preferred.
